I had a huge fight with one of my insurance companies about 5 years ago!  I kept at it, always asking to speak to the next supervisor up!  Anyway, LONG story short....they finally got sick of fighting with me and said "in an act of good faith" they would pay the claim.  I think they figured out I wasn’t going to go away!  Good luck, and hang in there!
