Question about virilization from a family member who doesn’t know CAH... What could this mean???

A relative who isn’t aware that there is CAH in the family had a baby girl a few month ago.  My sister-in-law who has a close relationship with her and spoke to her when she was still in the hospital related that she asked her whether she knows if it’s possible that doctors should have a hard time figuring out the sex of a newborn baby.  My sister-in-law answered that of course she did and asked her why she brought up this question but that mother just brushed her off and answered that she was asking out of simple curiosity.  If this wouldn’t be in our family it would be simple to believe that this was indeed the ,but since there is a case of CAH here it rang a bell. 

My sister-in-law is convinced that if that baby girl would have a sever form of CAH that mother wouldn’t be capable of hiding her emotions from her and in all these month there was nothing unusual about her attitude.  So what than could it be?  Maybe that child has NCCAH and the mother came across this information when she was doing research, but than, how would they detect the non classical form only two days after birth, or maybe, should I think this was indeed an innocent question?...  Any suggestions?...
