Our pediatrician actually had the pharmacy fill his RX when we were in her office because he was gagging and spitting up so much.  That just happened to be the time he was due for his afternoon cortef-lunchtime.  We gave them in her office at the same time.  She never said anything other than don’t feed him for 30 minutes after he had the Zantac.  The Zantac never worked he just finally outgrew it.  Now I have no use for all the cloth diapers and little hand towels we used to carry everywhere :).  Since he was on the Zantac only 2xs a day there weren’t many times that I gave them together.  He gets Cortef and Florinef at 430 am, Cortef at 1230pm, and Cortef and Florinef at 830 pm.  When he was on Zantac I gave it to him at 7 or 8 am and 5 or 6 pm.

You might want to get a second opinion on the cortef/florinef dosing schedule you are using.  I was told to dose 3xs a day at least 6hours apart.  I have always done it at 8hrs even in the middle of the night.  Also see if your doctor will set you up with a gastroenterologist just to make sure there isn’t something else going on.

On the comments about liquid Cortef.  We have a solution made up every Friday by my husbands childhood best friend who happens to be a pharmacist and also has a nephew with CAH.  We have never had a problem with his meds or with my son being sick.  His nephew however did have a lot of problems with the liquid cortef as opposed to the suspension that is made up.  We brush his teeth regularly and our dentist says that the suspension won’t decay his teeth.
