Eric I know we have ’chatted’ about your dose before and I now see that you take 40 mgs in the morning and 20 mgs at night, have you ever tried three doses a day? - although I must admit if it works for you and you have no issues with bone density or weight - why change it, however the 17 OHP level could then be brought down.  As said in previous posts cortef doesn’t last more than 6 to 8 hours in the body.  Have you have been checked for adrenal rests?  I suppose I just worry about the long term effects this high dose may have, however if you are metabolising quickly you may not suffer any problems.  Do you have any period of the day where you feel more tired- say early evening?
