New here--how to distinguish between CAH and ACTH secreting adenoma??

It was originally thought that I had congenital adrenal hyperplasia after my labs showed elevated 17 hydroxyprogesterone and testosterone, and borderline androstenedione.  But my ACTH stimulation test was normal.

Then they found a microadenoma of the pituitary, which was thought to explain my elevated prolactin.  Since then my prolactin has returned to normal.

I just got a second opinion and he ran more labs.  This was the first time I’ve had ACTH tested without stimulation.  It was 204.  I also have high pregnanetriol, almost 2000.

What should my doctor do to figure out if I have CAH, or if the tumor is secreting ACTH?  Thank you for any help!
