the endo can tell if the puberty starts from the ration of testosterone/  androstenedione. If both are high, it may be caused by adrenal under suppression. If testosterone is high but androstenedione is not, it may be caused by the puberty. In the latter condition,  the ovary starts to produce testosterone.
