It has your name on front (if you would like) and the engraving on the back of the heart. Also I have an Italian Charm bracelet where you can add the charms! It is real cool. It has a center piece on the charm bracelet with the medical emblem on it and your engraved information. 

Its a necessity to have the medical information on you at all times - and a way to give to the Cares Foundation.
