I have raised this issue before and got some good replies.  i am now at the stage of making the decision whether to take dexamethasone or not when planning for another child.  I am from Northern ireland and no one here has taken it before.  I wish to know if many women take dex in USA.  I have been doing some reading and am weighing up two issues:

unknown long term risks on foetus exposed to dex

psychological effects on an affected female in being exposed to androgens in the womb.

I already have an affected daughter, and we decided against surgery during infancy.  I have never found any difficulties in having a girl with CAH, and the physical aspect does not bother me.  As she gets older the virilisation is actually less noticable.  To have another child like her would be ok.  My main concerns are will she have difficulties in the future because of virilisation and exposure to androgens in the womb and therefore it would be better for me to take the dex for another pregnancy.  I really just want to do what is best, in the long term, for my future child.
