How about a design based on a sun symbol? The sun is up every morning with the hope and promise of a new day. The hope that there could be a cure for CAH (NIH ..THEY ROCK !)  The promise that a new parent that has a baby with CAH will feel comfort because they found the support of all of you. Or just a reminder to someone that has CAH and/or parents that the sun shines bright high in the sky and just because we have CAH doesn’t mean that we can’t shine bright and think highly of ourselves everyday also.

The designs for kids might be a little more straight forward but the designs for teenagers or adults could even get into more abstract designs.
