I also experience these fits with my Granddaughter who will be 4 in December. She is much better now than she use to be.. but she still has a melt down over some things. It does get better. But , I know when she was having them I was so frustrated because there was no amount of soothing her or consoling her. It just had to run its course. People would look at me like WHY DON"T YOU CONTROL THAT CHILD! Little did they know that I was praying on the inside that I wish I could. I honestly believe it has something to do with the meds or blood sugar or something... as soon as she was finished having her meltdown she would say she was sorry and be sooooo sweet and loving and then wanted to be soothed. I think that something is going on in there little system that causes them to act this way and they know they are out of control and it scares them as much as it does us. Both endo and pediatrics said it was not CAH related or her meds.  But , I have to ask myself..... How would they know? They don’t have CAH nor do they take the meds. I usually follow my gut instincts when it comes to things like this. The good news is ...it does get better!
