I would not withhold food  (maybe cut down on fats and sugars) but ask the doctor about the dosage of cortef and make sure that she is not over-dosed.  She may just be heavy because it runs in the family.  Under dosage of cortef can be very serious too and cause death or adrenal crisis (growth issues, feritily problems, etc).  The balance is very tricky with CAH.
