Hi Sherry:

I think this letter is perfectly acceptable.  I do letters like this all the time for both my mother and my mother-in-law when they have had to take care of my kids.  To be honest with you, they have both come to expect these types of letters because it helps them.  I also give them a copy of a letter my endo gave us if we ever have to make a trip to the ER.  It tells the ER personnel his Cortef doses and what to do in an emergency.  I also give a permission letter that states that they have my permission to make medical decisions on my behalf while they are watching my children.
