I have 2 SWCAH boys (6 and 7 years old).  One is starting 1st grade next week and the other will be starting 2nd grade.  Last year before school started, I talked to their teachers about their conditions and what signs/symptoms to look for and when they should call me.  This year I will talk to their new teachers, but I also want to pass out an information sheet they can refer to throughout the year.  Has anyone out their provided their teachers this kind of information?  If so, what information did you include?  I want to cover all of the important aspects of their condition, but I also do not want to give out too much information.  Any suggestions or examples will be greatly appreciated.
