Hi,

I have taken an information sheet to school with me ever year, and I am 16 going into grade 11, and I will be taking one again this year.

The type of information you should put on there is what CAH is, the symptoms, contact information of their Doctor and yourself, and also put the dosage of medication that your children take. If for some reason the teachers cannot reach you for whatever reason, they should call 911 and have an ambulance come pick your child up but that form MUST be given to the EMS on scene.

That is what I do, because I have been too close to death to risk it. No one ever knows what to do... it’s frustrating. Hope that helps.
