I am updating my daughters "letter to medical personnel" to reflect an addition from your letter.  Thank you!  I do not see a place for your Dr.’s signature.  Is there a reason?  Our letter is based on the one available from CARES in August of 2003 which we modified to suit our daughter’s needs.  Our letter starts out: To whom it may concern: This is a letter for our patient............. The letter ends with our endo.’s name and contact information and the following statement:  In the case of serious illness, _____ should be promptly evaluated by a pediatric endocrinologist and our office must be contacted immediately.

A little more than a year ago, this website was a huge help and a source of comfort.  Kelly @ CARES was even a bigger help!
