It really seems like he’s in puberty!  Maybe that was the cause of his behavioral problems.   I’ve been told that Lupron shots can be painful.  There’s also an inhalant called Synarel (I think) but it’s not really used much for kids.  Is he having emotional problems because of the early puberty?  It’s really alot to handle at age 9.
