We always give the missed dose immediately and then the next dose at the scheduled time.  It does happen occasionally, it is usually the 3pm dose, as things can get a little crazy in our house in the afternoons with 3 young girls running around.  Grace finishes school at 2.30 pm so I try to give her meds as soon as we get home.
