Jennifer, no problem repeating the info I have in my Granddaughters book. First of all it is just a three ring binder that have the slip in pages for sheets of information. On the Front of the book I have MEDICAL EMERGENCY ADRENAL INSUFFICIENCY with some clip art of a syringe and needle and pills. Page one ....... I have the Doctors list names/address/phone numbers and of the Hospital emergency number. Page 2...... Emergency letters from endo and pediatric complete with directions of amount of meds to give ect. Page3..... A small summary about adrenal insufficiency Front and back page. Page4........  Emergency instructions for Maritza Blake (granddaughter) and Treat Hypoglycemia immediately Instructions. Symptoms of adrenal insufficiency and instructions for mixing solu-cortef. Page 5......Patient education program on how to give  Intramuscular injection . Continued on page 6 Page 7...... Patient fact sheet Endocrinology and Congenital Adrenal Hyperplasia (CAH Literature) Page 8......... copy of birth certificate and an updated photo of Maritza . Page9....... copy of Social Security Card. Page 10... Immunization record. page 10....... Medical card and insurance information. page11.........  Food List .....I put in a list of updated foods she likes and dislikes and her favorites and juices and milk ect. eating habits,(grazer) Continues on the back with her developmental progress of dressing herself. play habits , Favorite toys, Nap and bedtime routine and our ways of discipline. I was told by her pediatric Doctor to have some additional authorization for emergency treatment forms already notarized .
