My husband and I were in a very similar situation about 3 years ago. His CAH had been managed fairly well for most of his life, but we moved and he went over a year without seeing an endo. During that time he became undersuppressed. When he finally went to a new endo, his levels were way off and his sperm count was really low (around 800,000/mL, average is 20-150 million/mL). His endo increased his dose of prednisone, but after 6 months his levels were still off so he started him on dexamethasone. About 2 months later his levels were better and his sperm count had increased to 3 million/mL. A month later we got pregnant. :) We now have a 16 month old little boy and are hoping to get pregnant again this summer. So, yes, it is possible for his sperm count to increase once his CAH is better managed. In our case it took about 3 months, but it may take a little longer since your husband hasn’t taken his meds regularly for so long. But there is hope! :)
