I was hoping that you responded.  You had mentioned in one of your posts that you worked for an insurance company.

 

My employer stats the change information was mailed to our home address.  I never received this.  We have to enroll online with our company and when selecting the PPO or EPO they did not list the carries of these plans, it simply stated PPO or EPO.  When selecting my medical benefits I pulled out my insurance card and it stated PPO on there, which is the reason I selected this one.

 

I had to address my letter to our corporate benefits department in NY.  They never gave me a name to address the letter to. I never thought about getting a letter from our endo.  Wish I would have thought about this before I wrote the appeal letter.
