You are right about the "1 in 4" chance, however it is 1 in 4 each time you get pregnant, therefore each child could be SWCAH or I’m sure there are some couples out there who are both carriers and never have a CAH child.   I have heard of a family where they had 3 nonCAH children and the fourth one was SWCAH.   Our eldest child has SWCAH and the next two do not have CAH.  We were lucky with the odds!  It really just comes down to luck I guess.
