Ours says:

Adrenal Insuffiency.  In emergency, needs stress dose Hydrocortisone.

Along with his ID # and the phone number, which would allow them to receive more detailed info. 

I also ordered a ’Medical Card’ from them; it’s the size of a credit card and has his medical record detail on one side and the emergency phone number and his ID # on the other side.  I punched a hole in the corner and attached it to the handle of his diaper bag.  This is mostly for his aunt who tends him during the day, so that she’d have the info available if she needed it quickly in an emergency.
