The ultrasound isn’t always definitive either - I was originally diagnosed with PCOS before being diagnosed with LOCAH, but the ultrasound could never get a good picture of my left ovary, where I frequently have pain.  My right ovary looked perfectly normal.

PCOS seems to be the diagnosis du jour for women these days.  I still don’t know for sure whether I have it or not, and many of the symptoms of it are the same as for LOCAH.  The ovary pain I have, though, indicates that I do have PCOS, but I was able to finally get a good ultrasound done of that left ovary about a two years ago and it looked perfectly normal.  Doctor was very surprised.

I guess I don’t worry about it too much at this point.  If the doctors want to say I have PCOS, then let them.  I KNOW I have CAH, and that’s a lot easier to clearly identify.
