Course of action with a broken bone with CAH kids

We were camping this weekend and on Father’s Day, my 5 year old nephew (non-CAH) tripped going inside the barn and broke both bones in his left forearm.  It’s a 40 minute drive to our local hospital.  The drive out of the camp ground was the worst due to bumpy roads.  His Mother and Father were both there and they rushed him to the hospital while we closed up camp and came home. 

If this would have been my 6 year old CAH-salt wasting son, what would have been the proper procedure?  example:  Give him the solu-cortef shot, splint break (or just leave well enough alone), apply ice, drive to hospital,  call endo in route???   Hopefully my husband and I would be together if it would ever happen. 

Should we just go the Children’s Hospital which is almost 1 hour away?

Any input will be greatly appreciated.
