We are in the same situation.  Gabe is 14 months old and very small.  From birth to 6 months he maintained around the 15% in height and weight.  At his 9 month check up he had dropped to below 3% in both height and weight (he weighed a bit less than at his 6 month appointment).  His levels were always OK and he was eating.  Gabe was put on a high calorie diet (wouldn’t I love to join him) and we reduced his Cortef a little bit.  He has been steadily gaining weight and getting taller.  We visit his endo in two weeks and I’m anxious to see if she is happy with his progress.  I think that he is currently at about 3% in height and weight, but at least it’s not continuing to drop.  (Also - we are not small people and our other son has always been 30-50%.)
