Yesterday my little sweetheart was vomitting, high fever and very sleepy, not herself at all. I had been double dosing and giving tylenol every 4 hrs but she was not feeling any better so I took her to our emergency at hospital as per our endos instructions. They did blood work. White blood count was 20, Dr on call thought this was a normal count for her due to CAH ??? At this point I asked her to check the previous records from past visits to emerg as I knew 20 is a very high white blood count and not the norm. Electrolites were checked and appeared fine. A urine sample was then taken and it did came back that she has a UTI. I asked for a hydrocortisone shot. I explained that a adrenal crisis can happen quick and that high fever, vomitting and excessive tiredness were signs of adrenal insufficiency. She was continuing to vomit and very larthagic. I was able to get her to drink small sips of gatoraid and she was not dehydrated. The Dr gave us a prescription for antibodics but refused the hydrocortisone shot as she said steriods were very strong drugs and she was not comfortable giving it to her. I DID HAVE A LETTER FROM OUR ENDO clearing outlining treatment, My husband explained that she took these drugs every day and they were life sustaining for her. The Dr still said she would not give the shot nor would she call in another Dr as she felt comfortable she was handling this situation. We left the hospital, came home and for the first time gave her the hydrocortosine shot ourselves. I had been nervous about this in the past but realized we had to do it. Quite quickly she perked up, no more vomitting and temp didn’t go up again. Antibotics have kicked in and she is feeling much better today. I am continuing to double dose. Our endo is 5 hrs from home so we have to rely on our local emerg. We have had to go to emerg in the past, have always had to strongly advocate for the Dr to follow protocal from Endo, but in the past have been able to eventually convince the Dr to give a shot of hydrocortosine. I am ranting but get so fusterated at lack of concern from Docs who have no knowledge of CAH. I am thankful we were able to give her the shot ourselves and now realize it is not hard to do..... I do however still want better care from our hospital. I have given our family Dr a copy of CAH book A Parent’s Guide and will also put a copy in the hospital Dr resource library, hopefully some of them will read it!!!Diana
