Dear Jessica,

If this is a doctor appt for Congenital Adrenal Hyperplasia then no you will not have a pap smear which is one of the ways to check for the different types of cervical cancer - not a perfect test when it comes to endocervical carcinoma but is better detecting squamous cervical carcinoma and is getting better with both because of the hpv thin prep testing .....medicine getting better all the time. Pap test once a year not every other year is great if you are sexually active. 80% of the  American public has HPV - can’t see it and men can’t even be tested - it’s only when partner comes up with cervical cancer or just the cells than they then know.

Most people bodies pass or fight of the virus (HPV) and it’s no big deal but some can’t and cervical cancer or cells pop up thus pap smear which aren’t always the best test but along with internal exams can catch the cancer early and are better now with the hpv thin prep need to ask for that one - this cancer supposed to come about after 40 but not true becasue any age can die. Condoms do protect but aren’t perfect and if you are trying to have a kid then well what can you do?

One can get HPV from the first person the are with to the 7th etc.  Really...You don’t know how many partners he as had either. If you have CAH and "the surgery" love that word .....vaginoplasty - makes pap smears a little harder to detect endocervical carcinoma because doctors aren’t perfect and sometimes the endo cervical component can’t ever be sampled and here is the rub the endocervical component doesn’t even have to be sampled to be considered normal ....75% of cervical cancers are squamous but 15% are endocervical - higher up next to the uterus and endocervical is on the rise and hpv isn’t the only factor in this for cevical cancer but it is the main one- also not having pap smears every year .......geez they said it was anemia, lower immune system for example from taking cortisones, having malabsorbsion like from celiac sprue or not taking in enough green veggie or folic acid like from orange juice .....yes I’ll give up OJ because the NP snickered diabetes from to much sugar....folic acid fights cervical cancer , fat around the middle lots of things that make up cervical cancer also low PH (bad eatting)   and lack of oxygen (no exercise). 

So, no pap smear at the Endo and really the endo will not go there ...not their speciality anyway do ask the endo about testing for the thyroid hashimotto’s thyroiditis because  that may cause infetility also besides other things (like cancer - I scared about another cancer)  and if you want also ask about insulin resistance a precursor to diabetes. The endo might send you for blood tests for the adrenal gland  (cah)  or other things. A Gyno or Primary Doc does pap smears for cevical cancer. 

I know of women in their 20’s, 30’s and 40’s that had cevical cancer- they had Husbands and some had kids but they are all dead from cervical cancer. They never get to see their children or enjoy life .....precious life.

I’m an endocervical cancer survivor and it will be two years at the end of June.  Life is beautiful.  

Sometimes I hate CAH and the focus on dropping a kid- we sliced her up with surgery and in the strive for normal? get pregnant - takes more than that to be a mother  .....oh dear i ventured off and here I’m guessing you are looking to see if you even have CAH? ok You’ll just go to an Endo office - fill out forms- he or she will give you a light exam check eyes, bp, or lungs, listen to your complaints and fill out blood work for you to get or they might do it on site if they have that set up. Then they will can with bloodwork from there and might have you come in for more testing. The they will set up a plan of action.  

It’s not a bad idea to list your concerns on paper.
