Hi Carol M,

Thank you for posting your knowledge and experiences about this.  I can’t believe it’s so difficult.  I would think it would be straightforward to just switch over from one to the other. So now i’m taking 30mg a day, my endo swears up and down that that’s the right amount. Yet 3 pharmacists each swear up and down that 0.625mg should be 20mg of hydro not 30mg.  This is crazy! yeah, i do feel really off, it’s less today though.  I just want to know if i should be taking 20mg, 25, or 30mg.  The CARES foundation says .75mg dex = 25mg hydro.  My pharmacists all say the same thing too.  Yet when i read about potencies, it says that dex is 40 times stronger than hydro.  Therefore mathematically 0.625mg x 40 = 25mg!

But you’ve gone through a transition so you know more about it than i do at this point which is why i’m reaching out to you all for assistance/guidance.  I’m waiting for my pharmacist to call my endo directly and discuss the conversion and get back to me tomorrow.  In the meantime i’m staying on the 30mg until my body if fully adjusted!

if .625mg of dex = 50mg of hydro and my bloodwork was normal.  How can 30mg end up giving me the same normal bloodwork results? How come no one is ever on 30-50mg of hydro? metabolized differently? All i’ve been hearing lately is how much closer to cortisol hydro is, and how much better it is in the long term.
