i know this sounds real harsh, but have you thought about getting your daughter committed, just until they can firgure out what is the underling problem.  i know you said your ins. would not pay for it, but most countys in the usa have county ones.  i can speak only about my own experince not taking my meds, but i felt 10 times worst with out my meds.  it is hard for me to belive she feels better without her meds.
