When my boys were diagnosed with hypoplasia vs CAH (they thought the boys could have 11beta), the big "give away" was completely normal androgen levels and 17 OHP levels prior to being medicated (and as they were collected during cort. stim test). The only elevated level (and it went through the roof ) my boys had was the precursor to aldostorone- I forget what it is called. And of course the fact that my youngest (11months) produces zero aldostorone, my older’s aldostorone levels are low as well, but it was determined that the boys Cortisol production was limited because during the stim test it only jumped 1 point. My older son had significant hyper-pigmentation on his hands and a long history of failure to thrive issues- always thought to be GI related. From the beginning they were confident my boys did not have CAH- they didn’t fit the normal profile. Not sure if that helps at all, but wanted to stress that my boys’ endos first thought was that the boys did not have CAH, but did indeed have hypoplasia- due to the normal levels of everything but cortisol and aldostorone.
