I do not have CAH, only a carrier, but this same thing happened to me.  I consulted a lactation consultant who advised me to nurse more and longer.  If the baby was not interested she told me to pump.  When I pumped initially, there was very little milk, but eventually it will build up.  She suggested trying to nurse every 3 hours (or more).  The more you do it, the more milk you make.  It worked.
