I have a teenage son with SWCAH, I think it is very important for these examinations to be done, the size of the testes are measured against the size of a bead to determine stage of puberty. Also it is important that the testes are felt for adrenal rests which appear as lumps which can happen with boys. However, I think it is very important that this test is explained to the parents and the child and done in a dignified manner, as hard as it is to accept, it is important. I do not believe that photos should be taken and see no necessity for this and we were certainly never approached, well done for saying no! I would question the doctor and be weary of seeing him again. It is important you do question as this may prevent others from facing the problem. Good luck.
