Does she sleep on her back? If she sucks her hands and if it is as cold as it has been here in Ohio, I would say the cold hands are from the combination of sucking her hands and the cold air at night. But as always, good to bring it to your ped. attention if it is a concern of yours.  

The saturated diapers are another story. My daughter that has the salt wasting CAH, used to have saturated diapers too. That could have something to do with her Florinef dosage. Every child is different so every medication regiment will be a tad different from one child to the other so it is hard to say, Well at that age my daughter was on such and such dosage. So that is definately something to bring to your peds. attention. Not that it is something alarmingly wrong. It just could be she is getting too much Florinef or too little, or it could be a normal kid thing and she is just drinking a lot causing her to pee a lot. But do bring this up to your ped. endo. the next time you visit him.
