Florinef dosing

My boys had their quarterly endo appt on Friday-  and the doc indicated that he was "concerned" about my 1 year old’s linear growth-  he is under the 5th percentile-  15th percentile for weight.  In early Dec we had increased his Corteff dosing by 2.5 mg a day-  (he had some labs that showed elevated ACTH levels, and had some pretty "patterned" vomiting(no other signs of adrenal crisis):  all these symptoms went away after the increase).  So we decreased my son’s dose on Sat-  The vomiting returned (always during a meal or snack)- so we went back to the other dose for the time being.  My question is this- the attending that was with my boys’ doc (he is a fellow) suggested that Johnny’s Florinef dosing might be off- so they did the blood work to check his rennin levels.  Johnny’s body produces negligible amounts of aldostorne- so I suspect that we will need to increase the florinef dosing- 

My question is this- has anyone had to increase the florinef dosing due to similar symptoms (ie vomiting)?  I forget to mention that around mid October we also decreased his Sodium supplementation - DR’s instructions- ) 

Have both emails and calls into the doc- was hoping to get some thoughts for others that have gone through this type of issue.
