Wow, I’ve been struggling with this for the past 4 years.  My 7 year old CAH daughter weighs 83 lbs.  We eat right, no junk, calorie restricted, but when she was about 3-4, she gained 35 lbs in 5 months.  She looks "puffy", but she’s also very stocky and muscular. Her levels were all normal and we’ve been trying to adjust her dosage and times of cortisol over the years to see if there are any changes.  Not really. She was taken off Florinef last year because they decided that she didn’t need it and that it might help with the weight-nope.  I get so frustrated because routine peds docs and teachers look at me like I’m a negligent parent with a couch potato kid-like I feed her McDonald’s and cookies all day.  Her father and I are both very thin and I’m constantly worried that this is going to be damaging to her in the future.  We’ve been in counseling for a few months because some kids have been tormenting her at school about her size.  I’m just trying to equip her with the tools she’ll need later in life to deal with it all.  I didn’t know how to even approach the issue with a 7 year old.
