Well if he doesn’t take his daily medications, and gets sick with some stomach bug or serious virus, he won’t have to worry about fertility. He will be dead. I hate to be blunt, but it has happened before. There was a young man with CAH that died because he too stopped taking his medication for personal reasons. He got a serious virus, and died. These pills that these patients that have CAH take are not just to surpress a certain horomone, they are vital to life if you have the salt wasting kind and get seriously ill.
