re: New to CAH and all confused
Oct. 5th, 2006   6:03am

Hi Iris,

First of all welcome to the CAH family and to this board!

My initial reaction to daily 25mgs of Prednisolone for one month is...ouch... that's way too high! However I know nothing about why your GP may have put you on that amount. Do you have a concurrent respiratory tract infection (URTI) or other problem happening at the moment apart from CAH?

If not, can I be bold enough to venture that it's not (generally) necessary to be on anything like that type of dose for CAH. It can cause other unwelcome problems. Did the endocrinologist you saw suggest a particular dose?

Let me give an example from my own experience. I have Classic CAH and a few years ago I had become non-compliant with my medications for some time. Eventually I became very unwell and needed to start back on medication regularly. After having testing done (before starting back on meds)  my 17-ohp levels were many hundred nmols/L (& many thousands of ng/dL) above normal range. The doctor then put me on 5mgs Prednisolone. This did nothing in bringing down the 17-ohp  and androgen levels, so for a couple of weeks he put me on 10mgs, then onto 7.5mgs Pred. This did help in bringing the levels down into a more acceptable level. So you can see that is a LOT less than you are on and it did the trick!

If you have Late Onset CAH (adult diagnosed) you will possibly have a more mild version than I have and therefore the dosing necessary to bring androgens under control is usually less. However I urge you  to somehow either go back/phone the endo or broach your GP with your fears about the high dose (assuming there are no other mitigating factors as I mentioned above).

Also you don't need to feel like a freak. You have been very fortunate to get to adulthood and have two children before symptoms have become so advanced.

Rosalie
