I think you will find that most people with CAH do not have a problem with gender identity. Some females may be born with ambiguous external genitalia, making identity of sex difficult initially but they have XX chromosomes and a normal reproductive system and generally identify as female. There have been papers written suggesting that the excess androgens produced pre-natally could masculinise the brain of CAH females and although some state this can lead to girls with the condition having more male orientated hobbies and some say there is a slightly higher incidence of lesbianism amongst CAH females than in the general population, this does not mean they reject their true biological sex and they still consider themselves female.
