4mgs is a terrific amount of dex. As Les says 80mgs hydrocortisone = approx 1mg of dex, so most adults only take 0.5mgs, with very few being on more than 1mg daily. Sounds to me as if you are being overtreated, which could be responsible for your weight problem. Are you under an endocrinologist and being properly monitored with the appropriate blood tests?
