Hi Joan,  My son also had trouble gaining weight initially (8lbs 10z born - 14lbs 2oz at 12 months!), so didn't even double his birth weight in a year. We also added duo-cal to his milk and feeds, but it did not seem to make any difference.  However we later discovered he had a high renin level and this was thought to have contributed to the problem.   After increasing his fludrocortisone, he gained weight at a  much better rate.  Has your niece had a renin test and is the level within 'normal' range?   If so and her doctor is happy that her hydrocortisone dosage is appropriate too, I wouldn't worry too much, she will gain at her own pace and once she is a bit older, will soon be demanding food she likes!  This doesn't seem to be unusual for babies with CAH - it is a 'problem' I hear time and time again but is always temporary!
