Hi Ali, Sorry to hear of your struggle to lose weight - it must be very frustrating! The problem with Dexamethasone is that it is a very strong steroid and 1mg is equivalent to 80mgs of hydrocortisone. Therefore when you are reducing your dose of dex, it cannot be done by very small amounts and I imagine that you probably could do with a slightly lower dose but due to the tablet sizes available, the reduction tried before was too high too soon, hence the side effects you suffered. In the USA they have liquid dexamethasone so can titrate doses more accurately but unfortunately this is not available in the UK
Have you discussed trying a different steroid to dexamethasone with your doctor (such as hydrocortisone of prednisolone?). As these steroids are not as potent, you may find that your overall steroid dose can be reduced by a smaller amount, in order for you to receive a more appropriate dose, allowing you to lose weight. I know some consultants favour giving both dex and hydro in order to achieve this, which is another possibility. It is certainly worth asking your endocrinologist about anyway.
