I had this problem myself about not being able to find any MedicAlert ones. As a child I always had the SOS kind. The thing that bothered me was that I have a couple of mates who are doctors and one is a nurse, when i mentioned to them about the MedicAlert necklace they said to stick to the SOS kind because they can be read straight away without the need to call or go online to find out peoples medical details. They say when someone is really sick making sure they have their meds is the main part and if they have to find out who they are it can waste time. . .
Although like you said if its international it seems silly not supply to the rest of the world, you would think that it would all be under one system with details.
