I think anyone with a life long condition may feel 'different' and feel your step daughter has probably not had the appropriate medical support to help her come to terms and deal with everyday situations, which come naturally to others.  CAH can be quite a burden and if she doesn't have any close friends to share her concerns and feelings with, she must feel quite isolated.  You are obviously a very caring step mum but sometimes it is easier to talk to someone outside the family about personal feelings.  I think she may benefit from talking to a psychologist, who understands about CAH and can help her move forward and make the best of her life.  At 21, she has a lot going for her, but needs to recognise this!   

Your stepdaughters medication does need looking at too.  Steroids are pretty powerful and it's important to ensure she is on an appropriate dose.  As before, I would recommend she requests a referral to a multi-disciplinary centre for endocrine conditions such as CAH, in order that she receives the best  all round care and attention, which she deserves.
