Someone with CAH has as much chance to change genders as someone with an untreated thyroid condition or someone without CAH. Someone not being treated for CAH and feeling off with lack of cortisol would also have the excess their excessive adrogens flowing from the adrenals but still in wouldn't have anything to do with the chromosomes of a person with CAH.
