If you look into the reason for the hydrocortisone therapy, it is to supplement your body with the missing cortisol and to also keep the androgen level suppressed.  The androgens (male hormones) are what is making these changes on/in your body.  Even with constant endocrinology support, many people with CAH have a hard time controlling their medication levels.  I am 28 years old and have had some pretty hard times in the last couple of years, and my levels have just been crazy!  I hope you are comfortable with your endocrinologist, and they are comfortable with you!  That can be a real problem for you if they are not experienced in dealing with CAH.  If possible, seek a second opinion.  It sounds like after three surgeries, you are getting pretty frustrated, and I am sure I would be too!  If I feel that my meds. are off, I take control and tell my endo. I want my labs checked.  Sometimes they just don't know if you don't tell them!  Good Luck!
