Wow, I can't believe your endo recommended a breat surgeon to you.  My endo doesn't even want me to ge a tattoo b/c of risk of infection.  when i asked her abot implants she looked at me like i was crazy.  My parents are totally against it too.  She said that would be a horrible risk to take.  b/c i'm sure you know any kind of trauma can lead to serious problems when you have CAH.  I have been controlled perfectly my whole life.  My endo said it would stupid to compromise my health for an elctive surgery.  She scared me into not getting them.  But i still really want to.  So now i'm wondering why your endo actually recommending surgery & mine is totally against it.  I'm 22 & never really developed breatsts.  Like you said its definatley not hereditary.  I'm small too (5'2 112lbs) but my breats look ridiculously small. I'm not even a full A. (32AA)  Finding bras, clothes, & swimsuits is impossible. My twin sister is very thin & does not have CAH & is D cup.  My mom is also very this & a C cup.   none of the women in my family have small breatsts they are all thin.  People don't understand what its like to have no breasts when you are in your 20's. Its not like they are just small, they are non exsisitant. I think a small B would be perfect.  But i can't convince anyone that surgery is a good idea for me. Good luck with your surgery.
