My daughter also suddenly had high cortisol levels at her last blood test.  We're going to test again next time.

Would you like to try something crazy?  I am probably a CAH carrier (don't know for sure) and definitely have a lot of diabetes in my family.  About 20 years ago I read a book about food allergies that fit me to a T.  I just started eliminating all these foods - everyone thinks I'm totally crazy - but basically, most of the foods I don't eat are carbs.  I don't have diabetes even though my father, grandmother, sister and now daughter either did or do.  I think it's my crazy diet.  I don't eat any grains - no wheat, corn, rice, barley, etc. or cane sugar.  I also don't eat dairy but I don't know if that's related to this.  Because your daughter has hives, maybe she has food allergies.  She could try the diet for a week or so and see what happens.  If she does do it, she could feel worse before she feels better so stick it out for a week.
