Symptoms of LOCAH: acne, hirsutism, muscle cramps, weight gain, fatigue, adrenal stress symptoms.  Of course, most of the common symptoms can also be attributed to PCOS.

I would say that your symptoms along with the shakiness and dizziness might be PCOS which can be caused by Insulin Resistance (you may get the shakes or feel a need to eat every few hours).  Of course, the chest pains and anxiety can be attributed to CAH sometimes.  The symptoms are very similar otherwise.  You probably need to check your 17-hydroxyprogesterone levels to be sure, and I would also check your Luteinizing Hormone:Follicle Stim Hormone ratio to diagnose PCOS if your ratio is over 3 (I think, that's right).
