I'm far from an expert, but my son was diagnosed with LOCAH based on high 17-ohp. When my daughter had symptoms - missing her periods, they diagnosed PCOS and not LOCAH based on the fact that she had normal 17-0hp and they also did an ultrasound and she did have cysts. I think you could have PCOS without having cysts but I'm not sure.
