Do I fit in here???

I'm wondering if there's a chance I could have LOCAH.  I found your board (love it!) & hope some of you can give me some insight...I'll try to keep this brief :)

Regular periods (probably never ovulated though- no PMS) til getting on the pill, never had a period when I got off. Dx with PCOS after ultrasound showed cysts. Metformin did nothing for me.  Ovulation induction with clomid & follistim worked, (3 times!) but unfortunately I never made it past the "chemical" pregnancy stage. So now my RE wants me to do IVF and I'm finally taking a step back & trying to understand WHY I'm not ovulating, & why I can't hang on to a pregnancy.  Got labs last week:

Not IR (normal insulin level, fasting glucose ~85)

total testosterone 69 

total/free testosterone? 3.8 (maybe this was just free testosterone?)

DHEA 18.7!!!

DHEAS 227

What do you think? Could I have LOCAH? 

Other symptoms: oily skin, minor break out occasionally but not really "acne", also mildly hirsute (b/w my breasts, abdomen, etc.)  I'm also underweight, always have been (5'10, 125#) and hypotensive (have orthostatic hypotension that comes & goes but can get pretty bad). Also, this is weird which I just put 2 & 2 together- my mom didn't have trouble getting pregnant but had cystic acne as an adult (hello androgens), same hair patterns as me, & her mom had trouble getting pregnant supposedly (took 5 yrs) & had same hair pattern.
